Welcome to our Self-Management Programs!

These programs are funded through a grant by the State of Tennessee to provide people who have chronic diseases, and/or their caretakers, with the tools they need to manage their condition.

Our Programs

Chronic Disease Self‑Management Program (CDSMP)

Support for adults living with any chronic health condition. Learn practical tools to manage symptoms, improve daily life, and feel more in control.

Tomando Control de su Salud

Spanish‑language version of CDSMP, offering the same proven strategies for managing chronic conditions in a culturally responsive format.

Diabetes Self‑Management Program (DSMP)

Designed for people with diabetes or pre‑diabetes. Build confidence in managing blood sugar, nutrition, medications, and long‑term health.

Programa de Manejo Personal de la Diabetes

Spanish‑language version of DSMP, providing diabetes self‑management skills in an accessible, supportive environment.

Program Qualifications

To qualify for one of these free programs, you must be an adult, residing in TN and self-identify as having (or be the caretaker of someone who has) a chronic disease, including:

  • Cardiovascular disease
  • Cancer
  • Asthma
  • Diabetes
  • Arthritis
  • Lung disease
  • Depression
  • Other chronic disease

The programs are offered in churches, community centers, other in‑person locations, and online. Participants will attend a 2.5-hour health education class once per week for six weeks. We will discuss information that can help participants better manage their condition and its symptoms.
