The Patient Information Guide is an informative booklet that answers questions you may have about your hospital stay. It is provided to every patient at Methodist and covers such topics as Safety, Patient Rights and Responsibilities, and Advanced Directives. Download the Patient Information Guide (PDF). 

Getting to Know Me

You may also find the Getting to Know Me form (PDF) helpful during your stay. It is optional for you to complete, but it will help your healthcare team get to know more about your personal preferences and what is important to you. The more we know about you, the better we can accommodate you and your family while you’re in the hospital.  Just give the completed form to your nurse and he or she will place it in your chart for the team to review. 

Doctor/Nurse Communication Tool

It can be difficult to remember what questions to ask of your nurse or doctor. They may appear busy, but it is also their job is to keep you informed of your progress and answer any questions you may have. It’s important that you and your family understand what is happening at all times. After all, you are part of the healthcare team.  You may find the Doctor/Nurse Communication Tool (PDF)helpful to better communicate with the rest of your team.

Partners in Care

When you are admitted to the hospital for your stay, the admissions team will ask you for the names and contact numbers of up to two Partners in Care. This informational sheet (PDF) will help you decide who to select as your care partners and what this means. Please bring their phone numbers with you.
