Office hours are:

• 8:00 a.m. to 8:00 p.m.  Monday through Friday  
• 8:00 a.m. to 5:00 p.m.  Saturday
• 10:00 a.m. to 6:00 p.m. Sunday 

Please contact the office as soon as possible if you cannot keep an appointment.

Bring a list of your medications when coming to your visit. This is important so that if changes are made, we can reflect these on your list.

Diabetic patients – please bring a log of your recent blood sugar amounts. If you do not have a log, we will provide one for you.
