Methodist Germantown Outpatient Rehabilitation
About Us
Our outpatient program features physical, speech and occupational therapists that can help you recover from a variety of injuries and manage medical conditions. If you are preparing for joint replacement surgery, we offer classes several times each month. We also host a monthly support group for stroke survivors and their caregivers.
Before Your Visit
Please download and complete the Medical History Form before your first visit. You may print the form and bring it with you to your appointment, or save it and email it to Teresa Luckett.
Location & Parking
Outpatient Rehabilitation Services is conveniently located in Medical Office Building A, a free-standing building on the campus of Methodist Le Bonheur Germantown Hospital. Parking is available around the building, or in the parking deck near the Poplar Avenue entrance.
