Application Requirements

Applicant Requirements:

The applicant must be a graduate of an accredited school of pharmacy and have completed a PGY1 residency, if applicable. Applicants must meet the requirements for Tennessee licensure and be registered to participate in the ASHP Resident Matching Program.

All Methodist residency programs should be submitted through the PhorCAS system. Any questions concerning the application requirements should be directed to the individual residency director.

Application Deadline:

All application materials must be received by January 10.

• Curriculum vitae
• Official copy of college of pharmacy transcripts
• Three letters of recommendation
• Personal interview
• Verification of completion of PGY-1 Residency, if applicable
