Appointments and Registration

The Methodist Teaching Practice is open weekdays from 8:00 a.m. until 4:30 p.m. and is closed on weekends and holidays. Appointments are required and can be made by calling 516-8785. If you are unable to keep your appointment, it is important that you call at least 24 hours in advance to cancel.

On the day of your appointment, please give your appointment slip to the medical receptionist when you arrive. If you have an address or telephone number change, be sure to give it to the receptionist. If you have any insurance coverage or are receiving welfare assistance, you need to bring the necessary cards with you as well as forms that need to be completed by our staff.

Be sure to tell the nurse if you have been to the Emergency Room or have been hospitalized since your last appointment.

Bring all prescription bottles or a list of all medications you are taking for the nurse and doctor.

Referrals

If your insurance assignment is to Methodist Teaching Practice, you must contact us for a referral before you schedule an appointment with us.

Forms

If you have forms that require your physician's signature, it is important to bring them to the Methodist Teaching Practice as soon as you receive them. Forms generally are not completed during patient hours. Please allow 1 to 2 weeks for completion.

Prescription Refills

Your physician may prescribe medicine for you. It is important to take this medication as directed. When you are out of the medication, please check your medicine bottle for refill information before calling the office. If refills remain, you may contact your pharmacy directly. If no refills remain, please have your pharmacist call us at 516-2966 weekdays between 8:30 a.m. and 3:00 p.m. If the pharmacist does not call, you must have the following information ready when the nurse or answering machine answers the phone:

  • Your name or the name of the patient taking the medicine
  • Your telephone number
  • Your physician's name
  • The prescription name, dose, and when you take the medication
  • The name and telephone number of your pharmacy

If your physician provides an order for additional refills, the nurse will call your pharmacy. Check your prescriptions regularly to avoid running out of your medicine when the office is closed. Prescriptions will not be refilled at night or on weekends unless it is an emergency.

The Methodist Teaching Practice is part of the Patient-Centered Medical Home (PCMH) program.  PCMH emphasizes the relationship between doctors and patients, with your doctor taking the time to get to know you as a whole person.
